Youth iPad and Laptop Access Opt-Out Request

The library provides iPads and laptops for children to use in the Youth Services area. Parents or guardians who want to opt their child out of independent access should complete this form. Once your form is submitted, the Youth Services Manager will send confirmation that your child's name has been added to the iPad and laptop opt-out list. This service is not available in the teen area.

I understand that by opting out library staff will not provide an iPad or laptop to my child unless (the parent/guardian) are physically present.

This agreement only applies to iPads and laptops made available in the Youth Services area and does not include technology available in the teen area.

I understand that this opt-out request does not guarantee my child will not gain access to an iPad or laptop from another patron.

I understand that I must contact the Youth Services Manager to remove my child's name from the opt-out list.

Garage Closure: The garage will be closed June 1 - September 1 for the youth remodel project. If you need handicapped access to the garage, call the front desk at 970-728-4519 x110.
